Where Do I Start?

I am the only person to take care of my 102 year old (yes, 102) grandmother. She had pneumonia this past winter and it was discovered then that she has a very bad heart. In the hospital, she was doing well until the heart ailment was diagnosed and then it seemed that everyone just gave up on her. I took her home and now have her well. She walks with the aid of a cane and can do most everything by herself. Since the hospital experience she has had a fear of losing me and the dependence on me is killing me. My life is not my own anymore. I can not be away from her for a minute without her calling for me night and day. She does need help for her bathroom needs, etc.. I really need some time off and don't know where to begin to find someone who would be willing to stay with her. How can I pay anyone to stay and get no sleep, day or night? She calls constantly, even when I'm sitting four feet away from her. Do I take her to a home or what ? Help would be appreciated. Thank you from the bottom of my heart.
