Elderly parents refuse help

I hope someone can help. My parents are both in their mid 80's and live alone in their home in Brooklyn, New York. My mother has severe and crippling arthritis and can barely use her hands. Two summers ago she fell and broke her hip. She refused to go to a rehab center after surgery, and therefore can barely walk, although she insists she can and has fallen many times since. My father, although he is in good health, is losing all his mobility because he does not get out either - they mostly just sit in their kitchen. He fears that she will fall again, and is therefore with her during their every waking moments. They are a united front in refusing all help. We were able to get a cleaning service, but now I believe it is time for them to be assessed for home health care. They refuse to move!!! My mother cannot bathe herself nor dress. I guess I'm mostly interested in getting someone in, if possible, to help her wash and dress in the AM, and do shopping for groceries once a week.

Can anyone suggest where I would begin? I work two jobs and live 65 miles away. I have a sister who lives about just as far, and a brother who lives in another State. We need help.
